Project Description Narrative:


Ensuring health and educational equity for children with chronic illnesses is crucial, especially as they face heightened risks for poor outcomes without adequate school-based support. Children from minoritized, low-income, and rural backgrounds often lack coordinated care between medical and educational systems, exacerbating disparities.

Project iSERVE aims to evaluate and improve the Educational Achievement Partnership Program by transitioning to a multi-tiered service model that tailors support to each family's unique needs. This approach will enhance service equity by assessing the reliability and validity of a new risk assessment system and comparing its effectiveness against previous comprehensive services.

By establishing more equitable and effective care coordination through iSERVE, Children's Wisconsin will improve health and educational outcomes for children with chronic illnesses across Wisconsin, particularly benefiting underserved populations. If successful, this project could serve as a model for statewide implementation and long-term funding support.
