Project Description Narrative:


Despite a long-standing health coalition and numerous successful community health improvement initiatives, Manitowoc County partners recognized that they were struggling to coordinate efforts across the county and lacking engagement of partners from all sectors and of the community. These missing connections meant that partners did not have a deep knowledge of existing coalition work, lacked awareness of how to engage the community in creating change, and duplicated efforts instead of building coordinated collaboration. After regrouping around a new coalition vision, mission and infrastructure, this project aimed to engage coalition and community members to develop five-year action plans and budgets to guide the collaborative work of Healthiest Manitowoc County (HMC) in their efforts to Achieve Healthy 25 and Achieve Healthy 75.

Project partners: Aging and Disability Resource Center - Manitowoc County, Aurora Health Care, Felician Village, Holy Family Memorial, Kaysun, Inc., Lakeshore CAP, Lakeshore Community Health Care, Manitowoc Activity and Nutrition Coalition, Manitowoc County Health Department, Manitowoc Injury/Falls Prevention Coalition, Manitowoc Mental Wellness Coalition, Manitowoc Public School District, Manitowoc Substance Abuse Prevention Coalition, Manitowoc-Two Rivers YMCA, Northeast Wisconsin Area Health Education Center, The Chamber, United Way Manitowoc County

Outcomes & Lessons Learned:


  • Developed five-year action plans to guide the Achieve Healthy 25 and Achieve Healthy 75 initiatives and engaged community members to lead six action coalitions to advance the plans into action for improved health in Manitowoc County Received the 2018 Community Collaborative Award from the Chamber of Commerce of Manitowoc County, a great honor and recognition of the work and dedication that went into the strategic planning process
