Project Description Narrative:

Wisconsin is home to some of the nation's worst health and healthcare disparities. With this hybrid event, the Wisconsin Collaborative for Healthcare Quality (WCHQ) will present and explore innovative programs, tools, and opportunities to improve health in Wisconsin and reduce healthcare disparities with health system and community collaborations.

Attendees of the regional event will learn about health system and community collaborations that are currently in place and social determinants of health, and will have opportunities to network. WCHQ will share the ideas and learnings generated at the event with their Health Disparities Improvement team to inform their continued work.

Outcomes & Lessons Learned:

Brought together 200 attendees representing 24 health systems along with community partners, corporate and payer partners, and governmental agency representatives to learn, collaborate and network on opportunities for reduction in healthcare disparities
Key presentations focused on UW Health's vision and strategic plan aimed at dismantling racism, Fort HealthCare's innovative school nursing program, Wisconsin Hospital Association and Unite Us on utilizing data and dashboards to connect patients to resources, evidence-based work of CHWs using the Pathways Community HUB model to reduce healthcare disparities and increase health outcomes for patients, and the Healthy Metrics partnerships recent reports describing data on health disparities released to date to drive health system work in quality improvement interventions
