Assessing residents’ attainment of ACGME competencies

The Accreditation Council in Graduate Medical Education (ACGME) has established the implementation of educational outcomes as an absolute requirement for accreditation of residency programs. The ACGME requires residents to demonstrate mastery in six areas (“general competencies”): 1) medical knowledge; 2) patient care; 3) professionalism; 4) practice-based learning; 5) system-based practice; and 6) communications skills. Each residency must develop and implement an outcome assessment system to document the achievement of each general competency.
The project team will develop a set of objective structured clinical examination (OSCE) stations related to each ACGME general competency. All medicine residents will be required to regularly complete a performance-based OSCE to assess the level of attainment of competency-based objectives, after which they will undergo competency-based educational training, including regular OSCE practice sessions with performance review, role-play, and small group learning. A final examination using the OSCE will be administered, and each resident will receive a final score for each station. Residence will have the opportunity to develop and enhance ACGME teaching and communications skills by serving as evaluators during OSCE sessions of other residents. The program will provide each resident with certification of attainment of each ACGME general competency-specific objective. The project will also provide information on areas of the residency curriculum that need improvement.
