Learning Event Support: Community and Healthcare System Collaboration to Reduce Health Disparities

Project Description Narrative:

Wisconsin is home to some of the nation's worst health and healthcare disparities. With this hybrid event, the Wisconsin Collaborative for Healthcare Quality (WCHQ) will present and explore innovative programs, tools, and opportunities to improve health in Wisconsin and reduce healthcare disparities with health system and community collaborations.

Attendees of the regional event will learn about health system and community collaborations that are currently in place and social determinants of health, and will have opportunities to network. WCHQ will share the ideas and learnings generated at the event with their Health Disparities Improvement team to inform their continued work.
