Project Description Narrative:


There are an increasing number of children between birth and three years of age with development delays or disabilities in Milwaukee County, where there is a disproportionately higher rate of development delay than the rest of the state and the U.S. While these children have had assessments and their local providers have kept records of them, data silos limit accessibility between providers.

Through this project, an integrated database for identifying and tracking children with developmental delays or disabilities birth to three years of age in Milwaukee County. The system will facilitate cooperation and data sharing among the partners and will be used to support agencies providing health services to these children.
