Project Description Narrative:

Riverwest Health Initiative (RHI) is a grass roots coalition growing from a shared purpose to improve the well-being of Riverwest residents. Working with community partners, RHI works to address health barriers including lack of adequate information and lack of input and prioritization into where and how health resource should be allocated for their community.

With this project, RHI will expand through a constituency building process; identify and prioritize community health strengths and needs through a constituency-building, community participation process; and complete their strategic health plan addressing priority health needs. Through these efforts, RHI will make a real difference in the health and well-being of Riverwest residents.

Community partners: Columbia St. Mary’s, UW-Milwaukee School of Nursing, SDC Community Partners, Franklin Pierce School, Peace Learning Center, UWM Riverwest Pierce Community Nursing Center, and OW Holmes Elementary School
