Reducing morbidity and mortality from chronic disease

Chronic diseases, including heart disease, cancer, and diabetes, are leading causes of death and disability in the U.S. and Wisconsin. In Monroe and Vernon County, death rates due to such chronic health conditions are higher than the rest of the state.
This project will address health risks for underserved rural communities in the targeted western Wisconsin counties by improving access to health services. Building upon the previously established CARING program, introduced by Viterbo University, the project will employ a community assessment model that builds links within the communities to ensure active participation in program initiatives by a wide range of community members and organizations.
