Maps are a valuable tool to illustrate relationships affecting health outcomes, public health risks, disease transmission, access to healthcare, and other public health concerns. As each Wisconsin local public health agency is required to conduct share its needs assessment process, Community Health Improvement Processes and Plans (CHIPPs) with local partners, maps can provide a visual representation of health data and statistics to support communication and analysis of population health measures.
Through this project, local health departments and community-based organizations will be provided access to standardized public data, GIS map creation tools, and a library of predefined maps that will transform community health improvement planning.
