In Wisconsin, current medical and social services systems don’t have means to ensure dementia patients and their families have timely access to the best resources for their needs. As the state population ages, the toll dementia care takes on caregivers, and its cost to our health care systems, will grow.
This project will promote the health and well-being of Wisconsin residents affected by Alzheimer’s disease and related disorders and their families, in particular those who reside in traditionally underserved rural and minority communities in our state. This will be accomplished by establishing Dementia Care Networks to foster quality care in target communities, and coordinating and integrating the delivery of medical and social services to patients at all stages of the disease.
