Increasing accessibility of health services to children and creating a continuity of care among providers

Communication breakdowns are common between primary care physicians, social services agencies, and families when it comes to the children in their care. Without strong communication plans and strategies, challenges such as poor follow-through, limited access to services, and potentially duplicated efforts can occur—especially for children with development delays and disabilities in the child welfare system.
This project will facilitate and streamline the communication process between agencies to improve the overall quality of care provided. Focusing on children with complex medical issues from diverse low-income families, project partners hope to create better long-term outcomes.
