Project Description Narrative:


Local census data shows that in Wisconsin’s Fox Cities, Hispanics tend to be poorer and less educated than the general population. This inequity results in a disproportionate rate of disease. Wisconsin Hispanics are also the ethnic group least likely to be insured, and the least likely to have a regular site for medical care.

This project will develop a culturally appropriate and effective patient navigation model to assist Hispanics with limited English proficiency with accessing and effectively using the healthcare system. Project partners will also identify specific strategies to strengthen the capacity of the Hispanic community to address healthcare issues.
