Creating a coordinated healthcare system in rural Wood and Marathon counties for older adults with chronic conditions

The majority of Wisconsin’s senior citizens have had one or more chronic condition, such as hypertension, coronary heart disease, diabetes, chronic obstructive pulmonary disease, and osteoporosis. Individuals with chronic conditions are more likely to experience functional declines and to be admitted to nursing homes and hospitals, an especially challenging issue for seniors in rural locations.
With this project, aging and disability specialists, and community health educators, and other community stakeholders in an evidence-based chronic care certification program, provide telephonic care management services to high-risk elderly, and referrals for in-home self-management services will be made. Ultimately, the goal is to create a coordinated and less fragmented system for older adults in Wisconsin, reaching them before major crises and encouraging them to actively participate in their care.
