In Wisconsin, diabetes claims the lives of twice as many African-Americans as Caucasians, while stroke proves 50% deadlier for African-Americans compared to Caucasians. Additionally, heart disease claims 20% more African-American lives than Caucasian lives. Unfortunately, 44% of African-Americans in Wisconsin battle obesity, the nation's highest rate, and 34% are smokers. Within Milwaukee communities where the Community-Based Chronic Disease Management (CCDM) set up a wellness site previously, at least 88% of single-parent families with two or more children struggled with inadequate income
Through this project, CCDM will implement a capacity-building strategy involving community churches. It will establish wellness sites in Milwaukee-areas with severe poverty and health disparities, collaborate with 30 African-American churches, and engage the community in wellness activities.
