Project Description Narrative:
Milwaukee County General Assistance Medical Program (GAMP) purchases health care services for uninsured, indigent Milwaukee County residents with a network of primary care clinics, hospitals and specialists. GAMP, a nationally regarded model public-private healthcare partnership, was established in 1997 when Milwaukee County’s public hospital closed.
This project will demonstrate the efficacy of chronic disease case management for certain established GAMP recipients who are patients at MCW Family Medicine Primary Care Clinic or General Internal Medicine Clinic. Project partners aim to improve patient knowledge, skills, health status, and health care utilization, transforming GAMP to a more patient and health-outcomes focused system of care for Milwaukee County’s indigent and uninsured citizens.
