Project Description Narrative:


Treatment and prevention for chronic diseases and conditions such as obesity, diabetes, and cardiovascular disease have experienced significant advancement over the last few years. The adoption of these discoveries into real-life practice continues to lag behind, particularly for underserved patients and populations, and the impact of these gaps on treatment outcomes is substantial.

Through this award, partners aim to advance the health of Wisconsin populations by improving the diagnosis, treatment, and adoption of best treatment practices of a number of chronic conditions relevant to people in southeast Wisconsin.

Project Updates:


• Successfully recruited two faculty to advance patient-centered outcomes research

• Furthered outcomes research through a small seed research grants program awarded through three funding cycles to a total of five investigators for $15,000 one-year research awards

• Documented completion of two seed research awards, including one that led to a new tool to help patients in decision-making to address infertility

• Garnered $2 million in extramural funding and more than 20 publications in scholarly journals
